Tubal Ligation on a 38 year old woman.
We use a "fallope ring" that is placed onto the section of fallopian tube and this causes it to disintegrate and scar, thus closing the opening for the passage of sperm. Without the sperm passage, a woman ovulates normally, but can not get pregnant. Her hormonal state is not affected.
